CREDENTIAL DEADLINE AND SPECIAL INSTRUCTIONS FOR CHURCHES/SET GIFTS

The final list of new candidates should be forwarded along with completed applications and photos to Credentials' office no later than June 15, 2024.

1. The list should include candidates' full name (first, middle and last)

2. The list should be categorized be Elder, Deacon, Minister, Mother, etc.

Completed renewal application(s) along with photo(s) are due no later than June 15, 2024. Please Note: No credential card will be processed without a photo. See photo requirement below.
